JOB SUMMARY:
The Discharge Coordinator, in collaboration with providers and nursing staff, is responsible for coordinating all aspects of the discharge process starting on admission for specific patient populations. This includes the creation of individualized patient and family education plans for discharge, coordination of teaching plans for patients and families, follow-up and coordination with nursing and the multidisciplinary team, and collaboration with case management for home care, durable medical equipment, discharge medications, and care needs.
MINIMUM QUALIFICATIONS:
Associate's Degree - NICU
Associate's Degree in Nursing while pursuing a Bachelor's Degree in Nursing will be accepted - all departments except NICU
1 Year of Experience
BLS for Health Providers (AHA)
CA RN License
PALS or PEARS (AHA) - within 6 months of hire

2 Years of Experience
The current salary range for this position is $59.73 to $97.42
A candidate’s placement in the range will be determined based on the terms and conditions of the applicable collective bargaining agreement.
